I'm looking for either a phone number or website (which they don't seem to have), OR if anyone has a custom order sheet they could get to me somehow that would be great. A phone number would do just fine though. Thanks much!

Best I can do to help you is....

Go to www.muskiesinc.org and find a contact for the St. Cloud, MN chapter. Mike was/is a member of that chapter. Contact someone from there and they will know how to get ahold of him.

Possibly Laura or one of the twin cities guys might have a phone number as well...over on 101.
