Location: Ohio | Hi,
There are a few ways to do it. I leave the blank in one piece. I bore a hole from one end of the blank to the other. I make the bend for the line tie in a vise and pull the wire through. I pull the wire through the bait, glue the lip in and then make the back hook hanger.
It's much easier to see how it done versus explaining it. I will post a video later tonight of how I do it.

Location: Ohio | Yes, the belly hanger is the same stainless steel wire that has a hook at the top to prevent the hanger from coming out. It's also epoxied in. The bait is one solid piece of wood, not glued.
