Since you are from Ontario, I'd try old Canadian companies; A L & W maybe.... AWS lures ...Hex lures etc.
Search around here....there are more possibilities, probably in Ontario Companies.

Someone told me they thought it might be a Creek Chub clone by one of the Pennsylvania lure makers
