Body style is similar to some Latianos I have seen....the "smile" is at a shallower angle though.
The 2 circles on the under side at the very front remind me of a jerkbait that had "eyes" in that location, can't remember the name though. This lure appears to have been painted over with flat black paint.
Here are 2 links to a couple of Latiano jerks, certainly not all of his lures would look like one of these. You will have to magnify these quite a bit to get the 'smile-angle' to show up. Hope this helps.

I think you may be right, look at the "smiles" on the L vs R, they are not symmetric cuts. Good catch.

The cuts actually seem pretty symmetrical minus the chip on the one side, the smile is just more shallow than anything I've found really. I haven't been able to find an example of a wooden sledge but it doesn't look like a leo on the back half of the bait. The thicker tail end looks exactly like some Latiano examples but the smile is different. I was considering having a repaint done.

It looks like a surprise cut style jerkbait and not a dive and rise cut...no? I would say it's a newman spraybombed in flat black.

Not sure Sledges were ever made from wood. They're thinner than that too. I'm holding a Leo in my hand right now that I stripped for a buddy last wknd and you can see the same small holes drilled out for lead, in the same patterm on the chin/belly. I'm betting it's a Leo. That looks like one of my paint jobs too!

That's a pretty common lead pattern on a dive and rise jerkbait, not really a distinguishing factor. This is a pic of an old Latiano. I would say the key components are A) the type of cut. B) whether it's cotter pins or screw eyes c) the thickness of the tail

The fat tail section reminds me of a Latiano copy that I bought a number of years ago from a certain tackle shop located between Ellwood City and Zelienople in PA. It was sold as a real Latiano, but I found out later it was really only a copy. I would say the paint job on your lure looks like black spray paint. It may not be the original paint job.

I'm glad it's sparked some discussion at least.

The action is dive and rise. It's got screw eyes, and it's definitely a spray bomb job. I was thinking about getting rid of it, but it seems like it has some good bloodlines I'm going to give it some time this year, we'll see if this dog can hunt !
