The little piece you are talking about is just a fitting that goes into the portion of the engine housing, and your hose attaches to it. Essentially, all it is doing is acting as a restriction of the water flow, which makes it shoot out....similar to what putting a finger or thumb over the end of a garden hose would do. If you choose not to replace it, you may want to get a longer length of hose which can fit through the hole and shoot out that way. The 2 stroke Johnson 40 hp and 50hp series did this...it was a gusher, which was always nice to see...It definitely would not have any issue with getting stuff caught in that little outlet hole due to that.
As long as water is flowing good and hard, you'll have no problem. If you decide to just use a longer hose, get a rubber grommet to go around it in the hole through the engine housing...this will keep the hose from slipping back in and will work as a seal around that location as well...
