After no luck trying two new SD cards, I went back to step one and re-installed the LCG-2000 update first. Then the LMS update auto-installed as advertised on the first attempt.

So, it appears I had a faulty LCG update that prevented the LMS v1.9 update from auto-starting. I distinctly remember receiving the "Update Sucessful" message on the first LCG update, but it obviously didn't 'take' due to a bad card or whatever.
