When making inline spinners how should I attach the second (middle) treble on larger in lines .? Also how to make sure they do not tangle in the tinsel skirts.

Plenty of ways to do this but I do it via split rings. As for tinsel tangle I like to put some shrink wrap on the hook with a touch of glue in the void.

I second the glue in the void to alleviate tangle. E6000 adhesive works great and sits in the void without running over.
Other ways to attach front hooks are threading them directly on the shaft thru the line tie or using a clevis like on George Wahl's original Eagle Tail. Great if you're using a siwash (single hook) and want them to hang a certain way.
Large clevises are strong but i back them up with a split ring also.
