Can anyone explain how to figure out moon overhead and underfoot? Is it simply just midway between moon set and moon rise, and moon rise and moonset?

There's also several charts/websites/phone apps/calendars that list it so you don't even have to calculate it.

Anyone have any insight as to why the Major and Minot times on the http://solunarforecast.com/solunarcalendar.aspx website differ from the times in the tables of John Alden Knight's Solunar book?
